All new clients please fill out the New Client Form. If you have more then one pet please fill out the New Patient Form for each additional pet. Once forms are submitted you can phone us at 306-764-3011 to book an appointment. A $65 deposit is required to book your first appointment.

Consent forms for surgery will only be accepted if your pet already has a surgery appointment booked. All surgical procedures require an exam at our practice prior to the surgery being scheduled.
